Accessing Your Insurance Benefits

 
     The information below will assist  you access your insurance benefits, should you decide to do so.   Also listed below are many of the insurance plans we service through participation on mental health/substance abuse treatment provider panels.

 

Questions to Ask Your Insurance Company

 
1. Is a referral from my  Primary Care Physician (PCP)  required before  beginning counseling?
 
2.  Is Dr. Baney a member of my insurance and/or mental health provider panel?
 

If so, what are my "in-network" benefits?

If not, what are my  "out-of-network" benefits, if any?
3. Do I  have a co-pay (fixed fee) or co-insurance (%) amount due at the time of service?
4. Do I  have a deductible amount?  Is it combined with medical claims?  How much of my  deductible has been met?  When does my deductible period renew?
5. Do my outpatient mental health/substance abuse benefits require pre-authorization of sessions?  [If so, inform Dr. Baney at your first session.]
6. Are there types of disorders, conditions, diagnoses, or treatment modalities excluded by my insurance? For example, must "medical necessity" exist?                 Is marital or family counseling excluded? Are there other limitations?

Insurance Panel Coverage at IPPS

      Insurance companies change frequently.  Be sure to call the phone number on the back of your insurance card to receive current information, or, consult your benefit administrator at your place of employment.   While we will be glad to consult with you regarding potential insurance benefits, final determination is made by   your insurance plan.
